Methylene blue first transforms into leucomethylene blue when administered intravenously as an antidote, and leucomethylene blue then changes the heme group from methemoglobin to hemoglobin. These results indicate that metHb-albumin clusters could be a more effective and safer antidote against cyanide poisoning than NaNO 2. Methylene blue is presently mainly used to treat methemoglobinemia. Furthermore, the oxygen carrying capacity was maintained in poisoned mice treated with metHb-albumin clusters and was low in those treated with NaNO 2. In mice subjected to cyanide poisoning, metHb-albumin clusters reduced mortality and alleviated metabolic acidosis, while maintaining the activity of cytochrome c oxidase in organs their efficacy was better than that of NaNO 2. In vitro cell experiments showed that metHb-albumin clusters prevented the cyanide-induced inhibition of cytochrome c oxidase activity, resulting in a strong cytoprotective effect. USUAL DOSE Adult and pediatric patients: 1 mg/kg IV over 5 to 30 minutes. Other formulations of methylene blue carry a warning that they have not been found to be safe and effective by the FDA. Spectral analyses proved that the metHb-albumin clusters possessed cyanide-binding properties similar to those of naked metHb. Antidote pH 3 to 4.5 ProvayBlue is a methylene blue formulation approved for use by the FDA. The metHb-albumin cluster is made by a metHb core wrapped by covalently bound human serum albumin. The standard intravenous dose is 1 to 2 mg of methylene blue per kg of body weight (0.1 to 0.2 mL/kg of a 1 solution) over 5 to 10 minutes, repeated in one hour if needed. Herein, we present a promising antidote: methemoglobin (metHb)-albumin clusters. Administer methylene blue antidote to patients who have cardiopulmonary symptoms. Liebelt, MD Abstract: Methylene blue has been in medicinal use for centuries and is best known as an antidotal treatment for acquired methemoglobinemia (MetHB). However, its use has serious drawbacks in terms of efficacy and safety. Methylene Blue An Antidote for Methemoglobinemia and Beyond Renita S. Sodium nitrite (NaNO 2) is a universal antidote for patients with cyanide poisoning. Methylene blue was reported to have been administered as an antidote in the USA 149 times in 2013 (116 in patients over 20 years old) by US poison centers 2.
